Innovative Solutions for People with Disabilities to Achieve Integrated Life Outcomes Grant

In 2016, STAR Services was awarded an Innovative Solutions for People with Disabilities to Achieve Integrated Life Outcomes Grant from DHS to partner with four disability waiver providers to improve people’s ability for relationship building and community engagement.  In 2016 and 2017, we had great success with achieving these outcomes with two partner agencies.  A third agency is scheduled to start in 2018, and we are looking for one additional agency to partner with in 2018.

The selected agency will receive training on how to help the people they support build relationships and become engaged in their community.  This training will be provided for the agency’s direct care staff, people receiving services, managers, guardians, and case managers.  Four people receiving support from the agency will receive person-centered planning to create an action plan on how to connect in their community and build relationships with others.  The selected agency will receive just over $11,000 in funding to dedicate one on one staff support time to help a person connect in their community and build relationships.  A STAR consultant will provide ongoing coaching and mentoring to the person and their support staff as the action plan is implemented.
